What is the "YOUR CHOICE" program?  The "YOUR CHOICE" program is a senior meal program offering adults age 60 and over, their spouse and/or dependent adult living with them, a hot nutritious meal for a donation. You choose when you eat, up to five-meals a week, breakfast, lunch, or dinner. "YOUR CHOICE" meals are now being offered at the East 10th Street Hy-Vee.

Nutrition is key! All meals provide a dietitian-approved choice of offerings from the five-food groups identified by the "program" label in the various ready-to-eat food cases.

How do I sign up? Meal tickets are available through The Center For Active Generations. Participants are required to attend a brief enrollment session where program details are explained. Your information is confidential. A suggested donation is necessary to activate your card. For the enrollment schedule, contact The Center For Active Generations at 2300 W 46th Street, Sioux Falls, SD 57105 or phone (605) 333-3304 between the hours of 8:00 to noon Monday through Friday.

Have you completed your National Aging Program Information System (NAPIS) form?
Your meal is sponsored in part by the National Senior Nutrition Project. We can offer your meals for a donation because the State of South Dakota and the Federal Government reimburse us for part of the cost. If you are 60 or over and wish to dine, for a donation, the Federal Government requires that we have a NAPIS form filled out by you and returned to us one time per year. Your information is confidential. If you do not have a NAPIS form on file with us, we have no choice but to charge you the full cost of the meal, which is $6.00 If you have not yet completed your NAPIS form for 2008, please contact our Nutrition cashiers, Susanne, Priscilla, or Rhonda for the NAPIS forms and assistance. Thank you for your cooperation.
